Before filling out a registration form, check you are in our catchment area opposite.
Patients can register with either the Princes Street or the Halkirk Surgery but usually register with the surgery nearest to them for convenience.

You may be asked for proof of identity (passport or driving licence with photo ID) and proof of address (any utility bill dated within last three months) so please ensure that you take this with you. See Registration Process above for further details on suitable ID. If you have a Medical Card, please bring this with you too.
You will need to complete one set of registration forms for each member of your household. Once registered, we require you to have a new patient check, consisting of height/weight measurements, lifestyle advice, blood pressure check and details of both patient and family history.
This initial check-up is usually performed by the Practice Nurse, and this, combined with the completed new patient questionnaire helps the doctor to assess your general health and to familiarise themselves with your medical history, while waiting for your full medical records to arrive.
